Coming home.
With a little more help.

Meals, everyday routines, and personal care support after a procedure. Let’s plan the practical help that makes coming home feel more manageable.

Let’s talk about care
Thoughtful support at home

Talk through the first days at home

Tell us what help your discharge team recommends, who will be home, and what your family can realistically manage. We’ll confirm which everyday tasks we can support.

  • Meal preparation and help around the home
  • Bathing and dressing support, as agreed
  • Company and practical help with the daily routine
  • Updates for your family and a coordinator to call

A clear plan before the front door opens.

Bring the discharge instructions to your care conversation. Nursing, wound care, and rehabilitation need the appropriate licensed professionals; we’ll clarify any additional services you need.

The Meet & Match Promise™

You and Mom meet the caregiver before regular care begins. There’s room to get comfortable.

Meet first

What help people actually need in the first week home.

Someone in the house for the parts of the day that are unsteady. Help to the bathroom and back, meals that appear without standing at a stove, the tablets taken on time, the follow-up appointment reached.

Our guide on the first week after hospital is the one to read on the ward, before discharge day arrives.

Is Medicare home health not enough?

It is a different thing, and families find this out late. Medicare covers part-time skilled care at home, such as a nurse or a therapist, when a doctor certifies she is homebound, and a home health aide only while that skilled care continues. It does not cover ongoing help with bathing, dressing and meals on its own.

That gap, between the therapist leaving and her being steady on her feet, is what this service is for. See what care at home costs, and how families pay for it sets out what each programme does and does not pay for.

How quickly can care start after a discharge?

Tell us the discharge date when you call and we will tell you plainly whether we can staff it, rather than taking the booking and hoping. If we cannot, we would rather say so while you still have time to arrange something else.

Which hospital is she coming home from?

It changes the first week more than people expect: how far the follow-up appointment is, what the discharge team sent home with her, who is nearby. We write that up per area, from Piedmont Atlanta and Northside to Wellstar Kennestone and Emory Decatur. Find her area in the areas we serve.

We come to her home across metro Atlanta, including Buckhead, Sandy Springs, Dunwoody, Brookhaven, Decatur, Marietta, Roswell, Alpharetta, Johns Creek and Smyrna. See the areas we serve for what changes locally, and tell us her address and we will say plainly what we can staff.
